All coaches and assistant coaches are required to have a completed background check on file with the MCAA dated within the last 2 years. If you have not yet had a completed background check and you wish to coach or assistant coach on a team please complete the following form, have it notorized and return it to the sport director.
We will do our best to set up a time prior to each season to make a notary available for you at a coaches meeting. If you are unable to attend this meeting you may complete the form on your own. You DO NOT need to pay for the background check, please return the notorized form to the address below and the MCAA will take care of submitting the form to the state.

The Milford Community Athletic Association (referred to hereinafter as MCAA) is a volunteer based community association. As such, each individual taking part in MCAA programs will be required to volunteer a set amount of time to fulfill the basic needs of the programs registered for. In doing so, the MCAA will take every step possible to develop affordable programs that achieve the purpose stated within the MCAA Articles of Agreement and By-laws:
The purpose of the MCAA is to provide Milford area youth with athletic activities that will promote sound bodies, keen minds, positive competitive attitudes, and the spirit of teamwork.
By accepting these terms upon registration, I (the registrant) understand that I will be required to adhere to the following MCAA volunteer policy:
I will perform four (4) hours of volunteer service per registrant in MCAA program, per season.
